
IOCC has 24/7 staffed resource homes located in Port Alberni for children and youth who may have experienced severe trauma and may have significant mental health, addictions and behavioral/cognitive challenges. We are expanding our business/opening new resource homes and always looking at hiring new team members.
Job Summary
Key Duties and Responsibilities:
1. Participate in the assessment, goal setting and program planning (PSP- Personal Service Plan) for individuals living in a residence. Documents, implements, and provides input into the evaluation of the program.
2. Provides the skills training such as meal preparation, housekeeping, personal care skills and personal finance and implements personal service plan.
3. Assist client with activities of daily living such as toileting, feeding, transfers, hygiene and grooming.
4. Participate in various client focused activities in accordance with care plan.
5. Recognize, analyze, and deal with potential emergency situations such as participants’ aggressive behavior to ensure no harm comes to the client and or the public. Reports problem to the Residence Coordinator or On-Call.
6. Administers medication to the participant in accordance with established policy.
7. Assists with case management by identifying potential problems and reporting any difficulties. Provide input to councilors and other professional, about development of appropriate program plans to achieve youths’ objectives. Contributes to the evaluation of participants’ progress and prepares reports.
8. Provides emotional support to the participants and their team of support, may include family.
9. Transports and assists participants to appointments, shopping or leisure activities.
10. Performs residence maintenance and housekeeping duties such as laundry, sweeping, mopping floors, changing light bulbs, inventory, shopping, cleaning equipment and food services.
11. Maintains reports such as statistics, logbook, daily activities on participants. Completes Incident Report forms as required and reports all critical incidents to the Residence Coordinator or delegate as well as Community Care Licensing.
12. Identifies social, economic, recreational and educational services in the community that will meet participants’ needs. Maintains liaison with other agencies, professionals, government officials and the community.
13. Performs other related duties as required.
