The Nuu-chah-nulth Tribal Council/Child and Youth Services is seeking a permanent, full-time Child and Youth Case Manager to provide services to parents of Nuu-chah-nulth children.  The purpose is to strengthen the wellness of families and children by meeting family’s health, social, developmental, and emotional needs.  Based out of the NTC Tofino office, this position uses a strength-based trauma informed and family-centred approach that will draw upon traditional teachings, stories and rituals related to health and wellness that have been long held by Nuu-chah-nulth-aht.  Travel by boat and plane may be involved.

Position Responsibilities include:

  • Facilitates the development and implementation of health/care and wellness plans for individuals that reflect the choices and priorities identified by their families   
  • Provides evidence-based educational materials relevant to the client’s care plan
  • Liaise with and assist family in planning activities in the home and community which support their health and wellness
  • Develop and facilitate educational workshops and groups (i.e., developing health and wellness goals) while collaborating with individuals such as counsellor, nurses, doula’s, etc.
  • Strengthens and supports families in their roles as decision makers and advocates on behalf of their children and themselves.  Assisting to advocate for families in various capacities such as education, employment, social services, MCFS, etc.
  • Explores and links cultural and emerging teachings, beliefs and practices related to health and wellness
Qualifications 

Preferred Qualifications:

  • Diploma in Human Services or Grade 12/GED plus other relevant education and experience
  • Strong communication (both verbal and written) and computer skills
  • Ability to work independently by managing time and workload effectively
  • Possess a compassionate, friendly personality, and possess a personal wellness plan
  • Knowledge Nuu-chah-nulth family systems and culturally appropriate practices will be considered an asset
  • Solid practice for respecting confidentiality is a must
  • Must have a car, valid driver’s license, clean drivers abstract and able to travel to NTC Central Region Communities
  • Provide acceptable references and criminal record check
